Dear Patient,
I am writing to let you know that I will be retiring from Carolina Cataract & Laser Center effective October 9, 2026.
It has been a privilege to be a part of your eye care, and I am grateful for the trust you have placed in me. While I will
certainly miss seeing my patients and colleagues each day, I am looking forward to this next chapter of life, where I
hope to spend more time with family and friends.
If you have any questions or need assistance during this transition, our team is here to help. We are happy to assist with
scheduling, medical records, or any other support you may need.
If you choose to continue your care with Carolina Cataract & Laser Center, you do not need to request or transfer your
medical records. Your records and medical history will remain available to your care team.
If you need to request your medical records, you can do so in any of the following ways:
- Online: https://www.carolinacataract.com/patient-information/patient-forms/medical-records-request/
- Phone: (843) 797-3676
- In Person: https://www.carolinacataract.com/locations/
If you have not already been referred to another provider, please contact us or use the following resources to help
identify a provider:
- American Academy of Ophthalmology: https://secure.aao.org/aao/find-ophthalmologist
- South Carolina Society of Ophthalmology: [email protected]
- Health Insurance Provider
Thank you for allowing me to be a part of your care. It is important to continue with your recommended eye care, as
delaying or discontinuing follow-up could allow certain conditions to progress and potentially affect your vision. I wish
you continued health and all the best.
Sincerely,
Alexander R. Kent, M.D.
